Summary:  Develop and maintain cooperative working relationships with social service providers and participants to assist participants in meeting their transportation and mobility needs.  Assist the Mobility Coordinator with Coordinated Transportation Plans and implementation strategies.

Essential Duties and Responsibilities:

  1. Initiate and maintain relationships with other departments, area social service and transportation providers to assist participants in meeting their transportation needs.
  2. Coordinate outreach and recruitment of participants in mobility counseling sessions and travel training.
  3.  Develop intake tools, materials, packets, and communication plan for intake.
  4. Conduct home assessments to complete supportive service referrals and mobility solution action plans that coordinate transportation services using community resources.
  5. Develop and complete follow up on the supportive service referrals including mobility solution action plan with the participant.
  6. Confer regularly with providers to assure follow through on identified needs in mobility solution action plans.
  7. Coordinate regularly scheduled cultural, social, recreational, and health and wellness programs focusing on improved mobility.
  8. Prepare, submit, and ensure the accuracy of required documentation, reports, referral tracking records, and participant files within established time frames.
  9. Prepare and conduct presentations on program progress for advisory groups.
  10. Other duties and responsibilities as directed by supervisor.






To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Education: Bachelor’s degreein Business management, transportation, social services or related field.  

Experience: One to three years’ experience working in transportation, business, or social services working with seniors and/or the disabled and one to three years’ experience working with word processing, spreadsheet, internet, and database software.

Additional Requirements:

  1. Initial and periodic Motor Vehicle Records checks, maintain a valid driver’s license, and maintain a safe driving record.
  2. Must have insured, dependable transportation available for daily use.
  3. Must submit to initial and/or periodic state and/or federal criminal checks.
  4. Must submit to initial and periodic drug and alcohol testing.
  5. Ability to be reached by cellular phone during business hours and after hours.
