Summary:  Responsible for the provision of home-based services as an adult educator to assigned family caseloads in compliance with performance standards, licensing requirements, and policies and procedures.

 Essential Duties and Responsibilities: 

  1. Recruit, enroll and maintain required caseloads according to the requirements outlined in the ERSEA plan.
  2. Collate information for health packets and ensure children are up to date with health services according to EPSDT.  Conduct health and vision screenings as needed.
  3. Visit families as assigned and ensure completion of the requirements outlined in Parent Family and Community Engagement (PFCE) and Early Childhood Development and Health Services performance standards.  Ensure appropriate referrals and follow up for identified medical and social service needs of children and families.
  4. Schedule 90 minute in-home visits weekly with children and their families per program requirements; make up visits cancelled per performance standards and written program procedures.
  5. Plan and facilitate Family Socialization days 2 times per month and assist with other group family events as outlined in written plans.
  6. Carry out assigned curriculum for child development in the context of home and during Family Socialization events promoting children’s progress and learning outcomes.
  7. Work with parents to develop home visits and individualized activity lesson plans based on development, screenings, assessments, and identified family needs.
  8. Complete initial orientation packets with each family assigned upon entering the home-based program.
  9. Assist families with accessing comprehensive services including health and dental and providing transportation as needed; Work as a liaison between families and community resources.
  10. Collaborate with families to establish family development goals and document the collaborative process in the family partnership agreements.
  11. Participate with assigned teams in reviewing educational information for assigned children and families.
  12. Educate parents and ensure completion of child’s health status including medical follow-up, physical health, dental health, mental health, and nutritional information.
  13. Complete accurate documentation of eligibility, enrollment, social services, medical and dental services, home visits, socialization experiences, and other relevant activities with families in each child’s file and in assigned database systems; Submit accurate reports to supervisor as requested.
  14. Build respectful, interactive, positive, culturally responsive, and trusting relationships with children and families.
  15. Assist in obtaining, documenting, and verifying the local share of in-kind donations.
  16. Other duties and responsibilities as directed by supervisor.






To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


Required Associate Degree social services, human services, family services, counseling or related field.


One to three years’ experience working with children and families and one year experience training and educating adults; up to one-year experience with word processing, spreadsheet, internet, and database software.

 Additional Requirements:

  1. Initial and periodic Motor Vehicle Records checks, maintain a valid driver’s license, and maintain a safe driving record.
  2. Must have insured, dependable transportation available for daily use.
  3. Initial and periodic physical exams required.
  4. Must submit to initial and/or periodic state and/or federal criminal checks.
  5. Must submit to initial and periodic drug and alcohol testing.
  6. Cellular phone for emergency use only.
